Adoption Agreement and Application
PLEASE NOTE: CPRFund is an all volunteer organization and adoption applications are processed in the order in which they are received.
You must allow at least 72 hours after submitting your application for a reply from one of CPR's Adoption Volunteers.


Adoption Agreement
We are looking for permanent, responsible homes for the animals leaving our care. Every year millions of animals are surrendered to animal shelters because owners allow them to breed indiscriminately or decide they no longer want the responsibility of caring for a pet. Dogs are not disposable objects. We realize that adopting a pet is a LIFETIME COMMITMENT. It is our hope that prospective owners are aware of this also. Therefore, our policies are strict for the sake of our animals.
     
  1. Any pet adopted from The CPR Fund MUST be spayed or neutered before leaving CPR care. CPR pays for the spay /neuter procedure at one of our affiliated vets. The cost is included in all adoption fees. If the pet you choose to adopt is too young for the procedure to be completed (younger than 8 weeks of age) you will be asked to place a deposit for adoption (half of adoption fee) at the time of your meeting with the foster parents. Once a deposit if received, the dog will be held for you until the procedure can be completed and no other applications will be processed for this dog.

  3. Any pet adopted from The CPR Fund MUST be kept up-to-date on all age appropriate vaccinations and preventatives for the rest of their lives . Every CPR dog 6 months and older is tested for Heart worms and either immediately started on monthly prevention if found to be negative or treated if they test positive. However, it is strongly recommended that you have your dog retested in 6 months time. It is possible for a dog to initially test negative while in our care if they have just been exposed to the disease. It is up to the new owners to maintain monthly heartworm preventative and follow up with their Vet. CPR Fund will not be held responsible should the dog test positive at a later date.

    THERE ARE NO EXCEPTIONS to the above requirements!!!!
    NOTE: If age appropriate vaccinations and preventatives are not kept up to date, the CPR Fund has the legal right to remove the pet from your possession without reimbursement of your adoption fee.

  4. Any dog adopted from The CPR Fund is to be kept as a house dog only. If you are interested in a large breed dog (over 50 lbs.) we suggest a fenced yard. No large or very active breeds will be adopted to apartments.
  5. No dog will be adopted to be chained outside or allowed to run at large. This policy is in accordance with Indiana State and local laws which require dogs to be restrained to your property at all times or on a leash.
  6. No dogs will be adopted for breeding, scientific experimentation, hunting, guard/attack use, or any purpose other than companionship.
  7. If you rent, we require written or verbal consent from the property owner before your application will be approved. All landlords WILL be contacted at the time of application processing.
  8. We do not adopt animals as presents or gifts without the potential adopter’s knowledge and approved application.
  9. You must be 18 years of age or older with a valid ID to adopt. In households with children, an adult must be responsible for the animal’s care. In households with more than one adult, we must have each adult’s consent before the application will be approved.
  10. CPRFund has a 7 day refund policy. If within 7 days of bringing home your new pet you decide it just is not working out, return the dog to the foster home and your adoption fee will be refunded.
  11. If at any time in the future you no longer feel you can care for you adopted pet, you must agree to first contact CPRFund and allow us the option to accept (or decline) the animal back into rescue. If quite some time has passed since the adoption and/or there are any behavioral or temperament issues with the animal we may not be able place them again into a foster home.
  12. Although all our pets are evaluated by a veterinarian prior to adoption, the CPR Fund cannot guarantee or be held responsible for any health, mental, or behavioral problems the pet may develop. Once you accept the pet into your care, YOU ARE SOLELY RESPONSIBLE FOR ANY AND ALL SUBSEQUENT VETERINARY CARE!  THE CPR FUND WILL NOT REIMBURSE THE NEW OWNER FOR ANY MEDICAL BILLS INCURRED AFTER THE ADOPTION IS FINALIZED.

Before submitting an Adoption Application. I will have READ, UNDERSTOOD AND AGREED TO FOLLOW THE POLICIES LISTED ABOVE.
